Exploring the Patient Page - Scores Tab - Assessment Cards

You'll find the Assessment Cards within each Patient Page's Scores Tab at the top.

An example of an Assessment Card

The Assessment Cards serve as a one-stop shop for information and actions related to the assessments a specific patient has completed including

  • assessment (and individual PROM) scores (raw and normalized) with the outcome (% change)
  • date assessment was last taken
  • assessment cadence schedule
  • ability to update the score without interrupting the follow-up cadence
  • view or modify the existing follow-up schedule for the patient's assessment
  • ability to pause an assessment
  • resend the assessment if the patient hasn't completed

An outcome is defined as the difference between two scores. In OutcomeMD, the outcome will always be the difference between the current and initial (baseline) scores.

Let's break down what you'll find in each card (or skip to the bottom to review in a quick video!)


At the top, you'll find the Assessment Category, Name of the Assessment, Normalized Score, and % Change.

  • This top area will show "Results Pending" if the assessment was recently started but has not been completed by the patient yet.
  • You'll also see a clock next to the Normalized Score if the follow-up has not been taken. This clock allows you to resend the assessment quickly.

Once your patient has taken a follow-up assessment, you'll see a % change (aka the outcome). All outcomes are the current score compared to the baseline,

  • If red, then the score declined.
  • If orange, then the score had no change (e.g., 0).
  • If green, then the score increased.


In the middle of the card, you'll find

  • one or many score bars under the top area (these correspond to the individual PROMs within each assessment)
  • sub-scoring for each PROM (if applicable)

At the bottom of the card, you can

  • view the date the assessment was last taken and the timeframe the % change refers to
  • customize the follow-up schedule for the patient (this will be green if the schedule has been customized, blue if it has not)
  • view the assessment schedule's cadence
  • update the score without interrupting the follow-up schedule
  • pause the assessment (or restart if paused)
