Exploring the Patient Page - Accessing a Patient Page and Editing Patient Info

Need to review a patient? You can easily access the Patient's Page by using the Search Bar within the top right corner of the navigation bar.

  • You will use the Search or Add Patient (name, cell or email) field. (See below)


Once you're within the Patient's Page, you can easily edit their information, if needed.

  • Click on the Pencil Icon next to their name

Within the Patient's Information module, you can add/edit their:

  • Name
  • Contact information (email and/or cell phone)

    Date of birth

  • Other identifiers (such as a Claim Number, Study Number. and/or Record Number)
    • If an Other identifier is added, then you can use them to search for your patient within the search bar!


You can also checkmark the box at the top right to NOT display the scorecard results page to the patient once they've completed their survey

  • Leave this box empty to ensure your patient will receive their score!


Cell Phone, Envelope and Target

  • The phone and envelope signify if you have the patient's phone number and email.
  • The target is used to add or remove the patient from the Care Management section

How to Copy Scores, Copy Q&A, and Refresh the Patient's Page

  • Within the Patient's Page, you can copy the most recent patient scores, and their most recent Q&A.
  • You are also able to refresh the patient's page (so you can see your patient's responses as they come in!)

How to document a Visit within the Patient's Page

  • You can check the box next to the Patient is in office to record and populate a Visit Symbol (V) on the Patient's Timeline for the current day.
